Do you require a physician referral?

If you would like to be seen by one of our doctors, we require a physician referral. Your current primary care physician, surgeon, or specialist physician can refer you to us.

Do you accept insurance?

Pain management services are covered by all major insurance plans including Medicare, Blue Shield of Michigan, Health Alliance Plan and other major commercial insurance carriers. We also accept Workers Compensation and Auto. Please contact our office and we will verify if your insurance is accepted.

Once an evaluation of your condition has been completed, the office staff will ensure that all necessary preauthorizations are obtained so that you can focus on your recovery rather than worrying about how your pain physician will get paid. Most patients have only a nominal co-payment.

How should I prepare for my procedure?

How can I pay my bill?

You can pay your bill in person at our office with cash, check or credit card. You may also mail payment to:
